After completing college at the University of Arizona, Dr. Reed attended dental school at the Oregon Health Sciences University (OHSU) School of Dentistry in Portland, Oregon, earning the DMD degree. As a fourth year dental student, he began working toward a Master’s Degree in Pharmacology. After completing dental school, Dr. Reed remained at OHSU where he continued to work toward the Master’s Degree, received advanced training in anesthesia and also completed a residency in periodontology.
Dr. Reed then returned to Tucson, Arizona to practice periodontics and did so until he retired from that area of dentistry in 2001. In 1998 Dr. Reed began to provide anesthesia services to other dentists in the state of Arizona and continues to do so on a part-time basis today.
Dr. Reed is on faculty at one teaching hospital and four different dental schools. The teaching hospital is Lutheran Medical Center based in Brooklyn, NY. There he is the Assistant Director, Advanced Education in General Dentistry (AEGD), Arizona Region and also serves as an Attending in Anesthesiology in the AEGD program as well as in Graduate Pediatric and Dental Anesthesiology Residencies.
Dr. Reed is a Clinical Assistant Professor, in the department of Endodontics, Oral and Maxillofacial Surgery and Orthodontics at the Herman Ostrow School of Dentistry of the University of Southern California and an Associate Professor in Residence at the University of Nevada, Las Vegas School of Dental Medicine and an Affiliate Assistant Professor in the department of Restorative Dentistry at The Oregon Health & Science University School of Dentistry. Rounding out the international flavour, Dr. Reed also serves as a Clinical Instructor in the Department of Dentistry on the Faculty of Medicine and Dentistry of the University of Alberta in Edmonton, Alberta, Canada.
Dr. Reed is Vice President of the American Dental Society of Anesthesiology (ADSA) and holds Fellowships in both the ADSA and Academy of General Dentistry (AGD). Dr. Reed is a Diplomate of the National Dental Board of Anesthesiology. His involvement in the ADSA continues as the Assistant Editor of Anesthesia Progress as well as serving on the Editorial Review Board of that journal. Dr. Reed is a journal reviewer for General Dentistry, the journal of the AGD and Special Care in Dentistry, the journal of the Special Care Dentistry Association and the Journal of the American Dental Association (JADA).
Dr. Reed is Chair of the Committee on Examinations, Committee on Ethics & Bylaws, Committee on Heidbrink Award and Committee on Annual Meetings and Programs of the ADSA. He is a member of the following ADSA committees: Strategic Planning, Research & Essay Awards and Membership & Component Development. Dr Reed chairs the American Dental Association’s Standards Committee Working Group 4.32 on Syringes, Cartridges and Needles. He is also a Fellow of the American College of Dentists (ACD).
As an author, Dr. Reed has written five textbook chapters, all in Sedation: A Guide to Patient Management. Two chapters were in the 4th ed. The first was entitled “The Physically Compromised Patient” and the second, “Neurologic Illnesses and Other Conditions.” Three chapters were in the 5th edition of the same text, “The Physically Compromised Patient,” “Neurologic Illnesses and Other Conditions” and “The Geriatric Patient.” He has further contributed to the professional literature by publishing numerous articles in peer-reviewed journals.
In what little free time he has, Dr. Reed enjoys being a flight instructor holding Advanced Ground Instructor, Flight Instructor, Instrument Flight Instructor and Multi-Engine Flight Instructor Certificates issued by the Federal Aviation Administration (FAA).

Doctor Malamed was born and raised in the Bronx, New York, graduating from the New York University College of Dentistry in 1969. He then completed a dental internship and residency in anesthesiology at Montefiore Hospital and Medical Center in the Bronx, New York before serving for 2 years in the U.S. Army Dental Corps at Ft. Knox, Kentucky. In 1973, Doctor Malamed joined the faculty of the Herman Ostrow School of Dentistry of U.S.C., in Los Angeles, where today he is Professor of Anesthesia & Medicine. Dr. Malamed is a Diplomate of the American Dental Board of Anesthesiology, as well as a recipient of the Heidebrink Award [1996] from the American Dental Society of Anesthesiology and the Horace Wells Award from the International Federation of Dental Anesthesia Societies, 1997 (IFDAS).
Doctor Malamed has authored more than 140 scientific papers and 17 chapters in various medical and dental journals and textbooks in the areas of physical evaluation, emergency medicine, local anesthesia, sedation and
general anesthesia. In addition, Dr. Malamed is the author of three widely used textbooks, published by CV Mosby: Handbook of Medical Emergencies in the Dental Office (6th edition 2007); Handbook of Local
Anesthesia (6th edition 2012); and Sedation - a guide to patient management (5th edition 2010) and two interactive DVD’s: Emergency Medicine (2nd edition, 2008) and Malamed’s Local Anesthetic Technique DVD (2004) (edition 2 - 2012)
In his spare time, Doctor Malamed is an avid runner, exercise enthusiast, and admits an addiction to the New York Times crossword puzzle, which he has done daily since his freshman year in dental school

Mort Rosenberg graduated from Tufts University School of Dental Medicine and then completed an anesthesia residency program at Tufts-England Medical Center where he was named Chief Resident. He immediately joined the full time anesthesia faculty and soon became Director of Anesthesia Clinical Services and Medical Director of Operating Rooms. A nationally recognized leader in dental sedation and anesthesia, Dr. Rosenberg has published extensively in the medical and dental literature. He has been on the Faculty of Tufts University School of Dental Medicine since 1975 and is currently Professor of Oral and Maxillofacial Surgery, Head Division of Anesthesia and Pain Control and Associate Professor of Anesthesia at Tufts University School of Medicine and Senior Anesthetist at Tufts-New England Medical Center Hospitals. He has been very active in the American Dental Society of Anesthesiology (ADSA) for nearly 30 years and is currently treasurer. In 2006, he was awarded the Heidbrink Award from the ADSA and delivered the Joseph Osterloh Lecture as well as receiving the Provost's Award at Tufts University for Teaching and Service.

After graduating from Meharry Medical College, School of Dentistry, Dr. Amanda Okundaye, DDS attended an Advanced Education in General Dentistry program with the Lutheran Medical Center as well as Hospital Dentistry program at the University of California Los Angeles. She realized that she had a love for anesthesia and decided to continue her education in a Dental Anesthesiology residency at the University of California Los Angeles. Dr Okundaye has a full time private practice solely dedicated to anesthesia for Dentistry in the States of Nevada and California. She has faculty appointments at the University of California Los Angeles department of Hospital Dentistry and Herman Ostrow School of Dentistry University of Southern California. She teaches IV sedation courses at the University of Nevada Las Vegas and Oregon Health Science University. Dr. Okundaye is an active member of American Society of Dentists Anesthesiologists and the American Dental Society of Anesthesiology.

Brian Chanpong, DDS MSc (Dental Anesthesia) completed his undergraduate dental degree at the University of Southern California and a three year hospital based residency in anesthesia at the University of Toronto. He is a diplomate of both the American Dental Board of Anesthesiology and the National Dental Board of Anesthesiology and is a certified specialist in dental anesthesia (Ontario). At the Faculty of Dentistry, University of British Columbia, he is the course director for local anesthesia and minimal sedation. As a member of the accreditation committee at the College of Dental Surgeons of BC, he is on the inspection team that accredits general anesthesia facilities for dentistry. Dr. Chanpong maintains an out-patient anesthesia clinic that provides general anesthesia and sedation services for dentists in Vancouver, BC.

Dr. Douglas W. Anderson Endodontist/Dental Anesthesiologist
Dr. Anderson was graduated from Lewis and Clark College in 1972 having spent 5 years as a undergraduate trying to get his fourth major in college, but missed it by a one term. He successfully completed three majors however in Biology, Natural Science, and History. After dental school in 1976 at the Oregon Health Sciences University School of Dentistry he enrolled at Portland State University as an undergraduate seeking another degree in English with the proviso he could step into the graduate English Masters Program the next year, while practicing general dentistry.
In 1977 a program at Oregon Health Sciences University and the Department of Anesthesiology in the School of Medicine was subsequently offered. The program is no longer in existence. After completing his tenure at OHSU, Dr. Anderson stayed on to join the department as an adjunct instructor part time while practicing general dentistry. He also completed a residency in Endodontics from 1983 to 1985, thereafter practicing both private practice endodontics and medical anesthesia at OHSU. In 1996 he retired from his endodontic practice and was appointed to the Anesthesia faculty full time until his retirement from academic life in 2007 as a Clinical Assistant Professor of Anesthesiology.
He has written chapters on Oral Facial Pain and the AIDS Patient, Dental Pain and the 5 Minute Consult (A book for emergency medicine physicians), part of a chapter on Legal Problems and Anesthesia covering Dental Problems and General Anesthesia, lectured on many aspects of dentistry, pharmacology, anesthetics both local and general, facial pain, neurophysiology of pain, wilderness dental problems, pharmacokinetics and pharmacodynamics. He was one of three authors rewriting the ADSA Enteral Sedation Exam, and the sole author rewriting the Parenteral Sedation Exam for the ADSA as well. He has also been a guest lecturer for dental residents on monitoring of anesthesia. Over the years he has taught medical students and oral surgery residents who complete their rotations through anesthesiology, the rudiments of operating room anesthesia practice and techniques of anesthesia.
Dr. Anderson is a fellow of the American Dental Society of Anesthesiology , a Diplomat of the National Dental Board of Anesthesiology, International Association for the Study of Pain, and the American Headache Society.
He has been a member of Medical Teams International, Medical Mission Ecuador, and Capitol City Medical Teams as an anesthetist and dental anesthesiologist for plastic surgery groups, pediatric orthopedic surgeons, and gynecology surgeons on medical missions to Peru and Ecuador since 2007 to the present.
Dr Anderson currently practices endodontics and dental anesthesia. He retired from doing medical anesthesia in 2007 after a 30 year faculty association with the School of Medicine, OHSU Department of Anesthesiology and Perioperative Medicine.
His private life is consumed with his wife Pam and their two children who are currently medical anesthesiology residents in training. He plays a classic guitar made by Woodly White, a former apprentice under Jeffry Elliot, is an avid skier, golfer, and writes.

Dr. Kenneth Lee graduated from the University of Southern California School of dentistry in 1989. He graduated with honors at the top of his class and received the Fixed Prosthodontics Award upon graduation for excellence in the field of fixed prosthodontics during the 4 year dental school curriculum. Upon receiving his D.D.S. degree, Dr. Lee entered the dental anesthesiology residency program at Loma Linda University Medical Center. After extensive training in the operating rooms of the dental surgery center and the hospital, he received his certificate in anesthesiology in 1990.
Dr. Lee is one of only a select few general dentists in the United States that is designated as a dentist-anesthesiologist. He is a fellow of the American Dental Society of Anesthesiology and is a Diplomate of the National Dental Board of Anesthesiology.
He currently holds the title of Associate Clinical Professor at the Herman Ostrow School of Dentistry at U.S.C., where he teaches I.V. sedation to dentists from throughout the United States. Dr. Lee teaches a select group of dental students at U.S.C., dental anesthesiology in the operating rooms at his surgery center in Sherman Oaks, CA.
Dr. Lee is co-owner of Solis Surgical Arts Center in Sherman Oaks, CA., the first multi-specialty licensed surgery center dedicated to the practice of dental anesthesiology.
He is co-director of Advanced Anesthesia Seminars, where he teaches dental anesthesiology to general dentists, periodontists, oral surgeons and leaders in the field of dental anesthesiology. He also teaches courses in advanced I.V. sedation, airway management and emergency protocol. Dr. Lee is an examiner for the general anesthesia and conscious sedation on site examinations for the California State Dental Board.
Dr. Lee maintains a private practice in Sherman Oaks, CA. He specializes in the comprehensive dental treatment of patients that need general anesthesia for their dental care.
In his spare time, Dr. Lee enjoys outdoor activities such as snowboarding, wake boarding and mountain biking.

Dr. Andrea Fonner was born and raised in the Seattle area. She graduated cum laude with a degree in Psychology from the University of Washington, and graduated with honors from dental school in San Francisco, California at the University of the Pacific. She then completed an intense advanced training General Practice Residency at the University of Washington where she became certified in intravenous conscious sedation and advanced cardiac life support.
In addition to private practice as a general dentist, Dr. Fonner was an affiliate faculty member at the University of Washington School of Dentistry and at Odessa Brown Children’s Clinic, an affiliate clinic of Children’s Hospital and Regional Medical Center in Seattle, WA.
While practicing general dentistry, her passion for anesthesia developed, prompting her to pursue an additional degree and career as an anesthesiologist. Upon entering the prestigious anesthesia program at UCLA, she received extensive training in anesthesiology including both hospital and in office anesthesia services. Her experience there also included comprehensive training in both adult and pediatric anesthesia modalities.
Dr. Fonner is a member of the American Dental Association (ADA), Washington State Dental Association (WSDA), Seattle King County Dental Society (SKCDS), American Academy of Pediatric Dentistry (AAPD), American Dental Society of Anesthesiology (ADSA) and the American Society of Dentist Anesthesiologists (ASDA).
Her dedication to education and the world of organized dentistry and anesthesiology has led her to the position of course director for the Review Course for ASDA. She is responsible for creating the course content and efficiently running this annual national meeting. She also is scheduled to lecture for the ADSA in the spring of 2012 at the annual meeting in Las Vegas. Finally, Dr. Fonner teaches in both the Pediatric Oral Moderate Sedation and Parenteral Moderate Sedation courses at USC.
She has also published several expert articles in national newsletters advising pre-dental students and served on various organized dentistry committees to help young dentists. In addition, she frequently gives sedation seminars to local dentists interested in sedation dentistry and incorporating anesthesia services into their own practice.
In her spare time, she enjoys running and spending time with her husband, Matt. Last year they completed the Seattle Rock & Roll Half Marathon!
Rick Ritt, BS, MA, EMT-PARAMEDIC
Rick Ritt has been passionately focussed in healthcare in a variety of settings, i.e. clinical, educational, service, and sales. As a paramedic for over 30 years, (13 as Paramedic Officer with the Chicago Fire Dept.), Rick has a vast foundation of acute critical care experience. Simultaneously, Rick has an extensive training history, as he directed the second largest American Heart Association CTC in the country. He was personally responsible for training 150,000 students in BLS, ACLS, and PALS. His AHA Instructor history includes Affiliate Faculty Status in BLS (25yrs.), ACLS (16 yrs.), PALS, (11yrs.). Also, his educational accomplishments include a BS degree from SIU, and dual MA degrees in Health Services Management and Business Administration from Webster University . In addition, Rick developed a great understanding of equipment/ service by establishing a parts and service distributorship with Laerdal, servicing Laerdal equipment in 10 midwestern states for over 10 years. In 1998 his company was acquired, where he served as National Education Director and Regional Manager. Rick first came to Laerdal 12 years ago serving as a Hospital Account Manager. For the past 11 years Rick pioneered the NY Laerdal Education Team and has been instrumental in the successful establishment, design, management, consulting and training of over 600 Simulation Sites in the US and Canada.
