Dr. Orr is an Oral & Maxillofacial Surgeon practicing in Las Vegas, NV., is married and has eight children.
 
Dr. Orr became an Eagle Scout at age 14, was awarded the Eagle Scout of the Year designation and scholarship by the Los Angeles Area Council of the BSA, and as an adult leader was honored as a Silver Beaver by the Boulder Dam Area Council.  Dr. Orr is the Physician on call for the BDAC and was a member of the BSA National Jamboree Medical Staff.
 
After graduating cum laude from Brigham Young University and with honors from the University of Southern California School of Dentistry, Dr. Orr completed residencies in Oral & Maxillofacial Surgery at Los Angeles County/USC/Medical Center and in Anesthesiology at the University of Utah Medical Center, where he also earned a Master of Science in Anesthesiology. Dr. Orr then obtained a PhD in Biophysics from Columbia Pacific University.  Dr. Orr subsequently graduated from William Howard Taft University School of Law and the University of Health Sciences Antigua School of Medicine. Dr. Orr is a Diplomate of the American Board of Oral & Maxillofacial Surgery, the American Dental Board of Anesthesiology, the National Board of Anesthesiology, and the American Board of Legal Medicine.
 
Dr. Orr is Professor and Director of Oral and Maxillofacial Surgery and Advanced Pain Control at the University of Nevada School of Dental Medicine, a Clinical Professor of Surgery and Anesthesiology at the University of Nevada School of Medicine, and also teaches Early Morning Seminary and UNLV Institute religious classes for the Church of Jesus Christ of Latter Day Saints.  He is a Forensic Odontologist for the Clark County Nevada Coroner’s Office and the Post-Mortem Coordinator for Nevada’s USPHS National Disaster Medical System.  Dr. Orr is a court qualified expert in dental and medical legal issues, and has published extensively in the professional literature.  Dr. Orr is a member of the Nevada State Board of Dental Examiners Anesthesia Committee, the Chairman of the Nevada State Society of Oral & Maxillofacial Surgeons Anesthesia Committee, and a past President of the NSSOMS.  Dr. Orr is a member of the Sports Medicine Team for the UNLV Athletic Department, the AAA Pacific Coast League Las Vegas 51’s (Toronto Blue Jays affiliate), and the PRCA National Finals Rodeo.

Mort Rosenberg graduated from Tufts University School of Dental Medicine and then completed an anesthesia residency program at Tufts-England Medical Center where he was named Chief Resident.  He immediately joined the full time anesthesia faculty and soon became Director of Anesthesia Clinical Services and Medical Director of Operating Rooms.   A nationally recognized leader in dental sedation and anesthesia, Dr. Rosenberg has published extensively in the medical and dental literature.  He has been on the Faculty of Tufts University School of Dental Medicine since 1975 and is currently Professor of Oral and Maxillofacial Surgery, Head Division of Anesthesia and Pain Control and Associate Professor of Anesthesia at Tufts University School of Medicine and Senior Anesthetist at Tufts-New England Medical Center Hospitals. He has been very active in the American Dental Society of Anesthesiology (ADSA) for nearly 30 years and is currently treasurer.  In 2006, he was awarded the Heidbrink Award from the ADSA and delivered the Joseph Osterloh Lecture as well as receiving the Provost's Award at Tufts University for Teaching and Service.

Doctor Malamed was born and raised in the Bronx, New York, graduating from the New York University College of Dentistry in 1969. He then completed a dental internship and residency in anesthesiology at Montefiore Hospital and Medical Center in the Bronx, New York before serving for 2 years in the U.S. Army Dental Corps at Ft. Knox, Kentucky. In 1973, Doctor Malamed joined the faculty of the University of Southern California School of Dentistry, in Los Angeles, where today he is Professor of Anesthesia & Medicine. Dr. Malamed is a Diplomate of the American Dental Board of Anesthesiology, as well as a recipient of the Heidebrink Award [1996] from the American Dental Society of Anesthesiology and the Horace Wells Award from the International Federation of Dental Anesthesia Societies, 1997 (IFDAS).
 
Doctor Malamed has authored more than 135 scientific papers and 17 chapters in various medical and dental journals and textbooks in the areas of physical evaluation, emergency medicine, local anesthesia, sedation and general anesthesia. In addition, Dr. Malamed is the author of three widely used textbooks, published by CV Mosby: Handbook of Medical Emergencies in the Dental Office (6th edition 2006); Handbook of Local Anesthesia (5th edition 2004); and Sedation - a guide to patient management (4th edition 2003) and two interactive DVD’s: Emergency Medicine (2004) and Malamed’s Local Anesthetic Technique DVD (2004) In his spare time, Doctor Malamed is an avid runner, exercise enthusiast, and admits an addiction to the New York Times crossword puzzle, which he has done daily since his freshman year in dental school.

After completing college at the University of Arizona, Dr. Reed attended dental school at the Oregon Health Sciences University (OHSU) School of Dentistry in Portland, Oregon, earning the DMD degree. As a fourth year dental student, he began working toward a Master’s Degree in Pharmacology. After completing dental school, Dr. Reed remained at OHSU where he continued to work toward the Master’s Degree, received advanced training in anesthesia and also completed a residency in periodontology.
 
Dr. Reed then returned to Tucson, Arizona to practice periodontics and did so until he retired from that area of dentistry in 2001.  In 1998 Dr. Reed began to provide anesthesia services to other dentists in the state of Arizona and continues to do so on a part time basis today.
 
Dr. Reed has an academic appointment at the University of Southern California  (Clinical Assistant Professor, Surgical, Therapeutic & Bioengineering Sciences, Department of Oral and Maxillofacial Surgery.
 
Since 2000, most of Dr. Reed’s time has been spent teaching at the Advanced Education in General Dentistry (AEGD) program in Arizona, sponsored by Lutheran Medical Center Arizona. There he is the Assistant Director, Advanced Education in General Dentistry, Arizona Region.
 
Dr. Reed is a member of the Board of Directors of the American Dental Society of Anesthesiology (ADSA) and holds Fellowships in both the ADSA and Academy of General Dentistry (AGD). Dr. Reed is a Diplomate of the National Dental Board of Anesthesiology. His involvement in the ADSA continues as the Assistant Editor of Anesthesia Progress as well as serving on the Editorial Review Board of that journal. Dr. Reed is a journal reviewer for General Dentistry, the journal of the AGD and Special Care in Dentistry, the journal of the Special Care Dentistry Association. He also serves on the Scientific Advisory Panel for the Journal of Endodontics.
 
Dr. Reed is Chair of the Committee on Examinations of the ADSA as well as chair the Continuing Education Committee. He also chairs the American Dental Association’s Standards Committee Working Group 4.32 on Syringes, Cartridges and Needles.
 
As an author, Dr. Reed has written five textbook chapters, all in Sedation: A Guide to Patient Management. Two chapters were in the 4th ed. The first was entitled “The Physically Compromised Patient” and the second, “Neurologic Illnesses and Other Conditions.” Three chapters were in the 5th edition of the same text, “The Physically Compromised Patient,” “Neurologic Illnesses and Other Conditions” and “The Geriatric Patient.” He has further contributed to the professional literature by publishing numerous articles in peer-reviewed journals.
 
In what little free time he has, Dr. Reed enjoys being a flight instructor holding Advanced Ground Instructor, Flight Instructor, Instrument Flight Instructor and Multi-Engine Flight Instructor Certificates issued by the Federal Aviation  Administration (FAA).
 
Dr. Okundaye was born and raised in Las Vegas, NV.  After high school she attended Florida State University (FSU) with an academic scholarship. At FSU she majored in anthropology and pre-dentistry. After college Dr. Okundaye decided to seek training in dentistry.
Amanda graduated from Meharry Medical College, School of Dentistry; Nashville, TN.  After obtaining her dental degree (D.D.S. - Doctor of Dental Surgery) she attended an Advanced Education in General Dentistry program with the Lutheran Medical Center; Tucson, AZ. There she greatly enjoyed practicing dentistry with the implementation of conscious sedation.  Amanda was then accepted to a Hospital Dentistry program at University of California, Los Angeles (UCLA). During her one year Hospital Dentistry residency she became even more interested in anesthesia and medicine.
Dr. Okundaye was then accepted to a two year Dental Anesthesiology program at the University of California, Los Angeles, where she completed her residency.
Dr. Okundaye joined the faculty at the University of Southern California (USC) in 2006 as a clinical faculty member in the Department of Oral and Maxillofacial Surgery and Dental Anesthesiology.
She is a member of numerous local and national professional organizations including the American Dental Society of Dental Anesthesiology, American Dental Association, American Society of Dentist Anesthesiologists and the Christian Medical and Dental Association.
 
 
 
 
 
 
 
 
 
 
                
Dr. Dave Carsten is a native of Washington state.  He earned a degree in Biochemistry from Washington State University and his DDS from University of Washington (1984).
He practiced restorative dentistry for 24 years and taught prosthodontics at Oregon Health Sciences University Dental School for 13 years.  He focused his last 5 years of practice on the treatment of chronic fear patients and IV sedation.  Dave is currently completing his year #1 of a Dental Anesthesiology residency at Lutheran Medical Center in Brooklyn, New York.  Dr. Carsten is a Master in the Academy of General Dentistry, a Fellow in the Academy of Dentistry International, and a Fellow of the American College of Dentists.
He was presented with the Award of Distinction in Continuing Education for 2006 by the Academy of Dentistry International.  Dr. Carsten has authored several papers regarding color perception.  He has spoken more than 100 times on various topics.  He looks forward to returning to practice in 2010 as a Dental Anesthesiologist.

Brian Chanpong, DDS MSc (Dental Anesthesia) completed his undergraduate dental degree at the University of Southern California and a three year hospital based residency in anesthesia at the University of Toronto.  He is a diplomate of both the American Dental Board of Anesthesiology and the National Dental Board of Anesthesiology and is a certified specialist in dental anesthesia (Ontario).  At the Faculty of Dentistry, University of British Columbia, he is the course director for local anesthesia and minimal sedation.  As a member of the accreditation committee at the College of Dental Surgeons of BC, he is on the inspection team that accredits general anesthesia facilities for dentistry.  Dr. Chanpong maintains an out-patient anesthesia clinic that provides general anesthesia and sedation services for dentists in Vancouver, BC.
 
 
 
 
 
 
 
 
 
George J. McAlpine DDS MS, Director, Dental General Practice Residency, University of Nevada School of Medicine
 
Dr McAlpine graduated from the Loyola University School of Dentistry, Maywood, Illinois in 1974. Upon graduation, he entered the US Air Force. He completed a two year General Dentistry residency in 1979 at the University of Texas, Health Sciences Center, Houston and Wilford Hall Medical Center, San Antonio, Texas. He is board certified by the American Board of General Dentistry and the Federal Services Board of General Dentistry. He was responsible for running two Dental Residency programs and three Air Force Dental clinics during his distinguished, 25 year career in the USAF. Dr McAlpine has been actively performing moderate sedation for 30+ years. Upon retirement in 1999 from Nellis AFB, NV, he entered private practice (briefly) in Tucson, AZ. Missing Las Vegas, he returned when offered the opportunity to become the Director of the Dental General Practice Residency program, part of the University of Nevada School of Medicine. Dr. Kenneth Lee is a graduate of the University of Southern California School of Dentistry. He received his Doctor of Dental Surgery degree in 1989. During his dental school education, Dr. Lee was selected by world-renowned educator Stanley F. Malamed, in a special 2-year program that trained him in intravenous sedation. While in this program, Dr. lee was also responsible for teaching local anesthesia, nitrous oxide sedation and CPR to fellow dental students.
Upon receiving his D.D.S. degree, Dr. Lee entered the general anesthesia residency program at Loma Linda University Medical Center. After extensive training in the operating rooms of the hospital and dental surgery center, he received his certificate in anesthesiology in 1990.
He is a fellow of the American Dental Society of Anesthesiology and is a diplomat of the National Dental Board of Anesthesiology. He is currently an assistant associate professor at the U.S.C. School of Dentistry and teaches I.V. sedation to dentists throughout the world. He continues to train Dr. Malamed’s  I.V. sedation dental students at Solis Surgical Arts Center, an offsite teaching facility for the U.S.C. School of Dentistry.  He is also co-director of Advanced Anesthesia Seminars, where he teaches general dentists, periodontists, oral surgeons, medical and dental anesthesiologists advanced I.V sedation, airway management, and emergency anesthesia protocol. He is an examiner for the onsite conscious sedation and general anesthesia accreditation program for the State of California.
He maintains a private practice where he specializes in dentistry under I.V. sedation and general anesthesia.  He is co-owner of Solis Surgical Arts Center, a multi-specialty surgery center dedicated to the delivery of dental care under anesthesia. He has a passion for the comprehensive and esthetic restoration of patients requiring sedation and anesthesia. He divides his time in practice utilizing the  operator-anesthetist model and separate dental anesthesiologist model.  He has treated thousands of patients under I.V. sedation and general anesthesia.
 
Dr. Caputo has a private practice limited to anesthesia for dentistry providing office-based care for pediatric, special needs and adult patients in Tucson, AZ. He is a Diplomat of the American Dental Board of Anesthesiology and the National Dental Board of Anesthesiology. He holds a consulting staff position as two area hospitals and serves as an Assistant Program Director in Arizona for Lutheran Medical Center's Advanced Education in General Dentistry Residency. Additionally, he has teaching positions at the University of Colorado Health Sciences Center, School of Dentistry and the Arizona School of Dentistry and Oral Health. He has served for ten years on the Board of Directors for the American Society of Dentist Anesthesiologists and is currently the Immediate Past President.
